- Pays first $250 of expenses per in-hospital admission and
- $30.00 a day for room and board. $60.00 for intensive care
- 20% of the next $12,500 of eligible expenses
- Ambulatory surgical center benefit
365 Plus
Option I - $100 fixed daily benefit Option II - $200 fixed daily benefit
- Home Health Care Benefit
- Convalescent or skilled nursing facility
