                                                           Eligibility

All active, permanent employees under the age of 70 who work 30 or more hours per week in an agency participating in this Plan are eligible. Spouses and dependents are ineligible.

 

When To Enroll

Within 60 days of employment with the State of Florida

(see list of approved agencies)

OR

During the annual open enrollment period.

                                                                   OR

With health statement and the proper application submitted to Capital Insurance Agency.

 

How To File a Claim

• Claim forms can be obtained from your local Capital representative or call  1-800-780-3100, or email us , and we will mail the form to you.
• Complete all parts of the claim form, including your physician and employer.
• Mail the claim to the address on the form.
