How to Enroll

The State of Florida offers eligible employees a number of optional supplemental insurance plans in which to participate. These plans include: hospital supplements, cancer/intensive care , dental, vision, and disability. Premiums for these plans must be pre-taxed, which means the premiums are deducted before Federal Income or FICA taxes are calculated.

Capital Insurance Agency offers the following supplemental programs on a pretax basis:

• Cancer and Intensive Care Coverage – Underwritten by AFLAC
• Dental Care Coverage – Underwritten by CIGNA
• Hospital Supplements – Four plans from which to choose, depending upon whether you have the state health insurance or an HMO:
• 30/20+ or PPP
• 365+ or SIS Plan
  Underwritten by Alta Health & Life Insurance Company

How to Enroll

1. INTERNET ENROLLMENT:
Complete the company application on this website by clicking on the Hospital Supplements, Cancer Intensive Care, Dental Insurance. Print out a copy of the application and fill out. Proceed to People First Service Center and follow instructions or call People First at 866-663-4735. Mail your company application to Capital Insurance Agency, P.O. Box 15949, Tallahassee, FL 32317-5949.

2. PAPER ENROLLMENT:
Obtain a company brochure (can be requested through People First Service Center or by downloading and printing out the brochure online — click on Hospital Supplements and/or Cancer Intensive Care.) This brochure contains the company application. Complete the Official State Enrollment Form and the company application and mail State Enrollment to People First and company application to Capital Insurance. You may obtain these forms through the People First Service Center or by contacting Capital Insurance Agency.

NOTE: Coverage is contingent upon BOTH the company application and the premiums being received. AFLAC requires underwriting before coverage is approved.
