How to Apply

The State of Florida makes available a number of optional supplemental insurance plans in which eligible employees can participate. These plans include hospital supplements, cancer/hospital intensive care, dental, and vision. Premiums for these plans can be pre-taxed, which means the premiums are deducted before federal income or FICA taxes are calculated.

Capital Insurance Agency offers the following supplemental programs on a

pre-tax basis:

• Cancer and Hospital Intensive Care Coverage
• Dental Care Coverage
• Hospital Supplements
• HumanaVision

                        How to Apply

      INTERNET ENROLLMENT:

1. Download and complete the company application, if required, found by clicking Cancer/Hospital Intensive Care.
2. Mail your company application to Capital Insurance Agency, P.O. Box 15949, Tallahassee, FL 32317-5949 and proceed to No 4 of the instructions.
3. To enroll online for the vision, hospital supplement, cancer, hospital intensive care, or dental plans, proceed directly to No 4 of the instructions.
4. Proceed to People First Service Center and follow the instructions or call People First at 1.866.663.4735.

NOTE: Coverage with Aflac is contingent upon BOTH the company application being received and approved by Aflac

the proper election with People First.
