- Choose from 3 Elimination Periods to find a plan that suits your personal needs and your budget.
- Evidence of Insurability waived upon initial eligibility for enrollment; required if enrolling after initial option to enroll is not taken or if making a change to coverage.
- 60% of pre-disability income up to the maximum benefit of $5,000 Monthly (minimum Monthly benefit $100)

HOW TO ENROLL
Enroll during Open Enrollment or as a New Employee (within the first 60 days of employment). Please take a moment to review the Brochure for a full outline of the benefits available to you. Complete the Enrollment Form and return it by mail to Capital Insurance Agency’s Group Benefits Department, P.O. Box 15949 Tallahassee, Florida 32317.
