Humana Dental Has You Covered!
Humana is pleased to offer you two dental plans to choose from this year. While some of the benefits are similar, others are distinct to each plan. Be sure to review the features in this book to make the right choice for your dental health and budget.
PLANS TO CHOOSE FROM ARE:
- A managed care plan, HD205 Prepaid.
- An indemnity plan, Schedule B.
HOW DO THE PLANS WORK?
HD205 Prepaid covers preventive care and other dental procedures as listed when you’re treated by your selected primary care dentist. If your dentist decides you need more specialized treatment, you’ll be referred to a participating specialist. With the HD205 plan, the participating specialist’s fees may be discounted at 25 percent. General dentistry and specialty services are available only in areas where Humana has a participating general dentist and/or specialist.
Schedule B covers preventive care and other dental procedures as listed when you’re treated by any dentist you choose. Patients will be reimbursed the dollar amount shown upon completion of the dental procedures listed. Each covered person will be reimbursed up to the Calendar Year Maximum of $1000.00. Procedures not listed on the schedule may be charged at the Dentist’s “Usual and Customary” Fee.
HOW TO ENROLL
Enroll during Open Enrollment or as a New Employee (within the first 60 days of employment). Please take a moment to review the Brochure for a full outline of the benefits available to you for Deduction Code PTB 103. You must contact the People First Service Center, toll-free 866.663.4735, or access their web site at https://peoplefirst.myflorida.com to make your election. Enrollment will not be complete unless People First is notified.
