LIFE INSURANCE PLAN FOR DBPR & FDLE EMPLOYEES
SPECIAL FEATURES OF GROUP TERM LIFE INSURANCE
GUARANTEED ISSUE COVERAGE During the Open Enrollment period employees under 60 years of age can be insured for $10,000 with no medical questions asked and up to $500,000 by answering four broad health statements. The same coverage of up to $500,000 can be obtained by the spouse by answering the four health statements.
NEW EMPLOYEE COVERAGE If you are new to your Department (within 60 days) you can receive $100,000 coverage with no medical questions asked.
SPOUSE AND DEPENDENT CHILDREN Spouse and Dependent Children can be insured with or without the employee being insured. Employee or spouse must have coverage to cover children.
COVERAGE IS PORTABLE Should you leave State employment you may continue to keep your coverage.
DEPENDENT CHILDREN CAN BE COVERED FOR ONE LOW PRICE Example: for $1.99 all dependent children can each have coverage of $10,000. The premium is the same regardless of the number of children.
LIVING BENEFIT & WAIVER OF PREMIUM Living Benefit & Waiver of Premium are included at no extra charge. ** To be eligible for this plan, an employee must be actively working in a position that requires 30 hours of work or more per week. Those persons working on a temporary basis or filling an OPS position are not eligible for enrollment. Reliance Standard Life currently covers employees in twenty-eight departments of the State of Florida. Make certain you take a moment to review your brochure.

HOW TO ENROLL
Enroll during Open Enrollment or as a New Employee (within the first 60 days of employment). Please take a moment to review the Brochure for a full outline of the benefits available to you. Complete the appropriate Enrollment Form (DBPR or FDLE) and return it by mail to Capital Insurance Agency’s Group Benefits Department, P.O. Box 15949 Tallahassee, Florida 32317.
