- Pays first $250 of expenses per in-hospital admission and
- $30.00 a day for room and board. $60.00 for intensive care
- 20% of the next $12,500 of eligible expenses
- Ambulatory surgical center benefit
365 PLUS
Option I – $100 fixed daily benefit Option & II – $250 fixed daily benefit
- In-Hospital Stay
- Convalescent or skilled nursing facility
- Ambulatory surgical center benefit
STATE INSURANCE SUPPLEMENT (SIS)
- Pays first $100 of eligible expenses per Hospital Admission.
- 10% of the next $25,000 per calendar year
- Ambulatory surgical center benefit

Enroll during Open Enrollment or as a New Employee (within the first 60 days of employment). Please take a moment to review the Brochure for a full outline of the benefits available to you for Deduction Code PTB 101. You must contact the People First Service Center, toll-free 866.663.4735, or access their website at https://peoplefirst.myflorida.com to make your election.
